NOTES:
A $50.00 fee is required for Saturday parties, to be paid before the date of your event. The $50.00 fee only applies to regular club nights and hours. This fee does not apply to special events. We require you to treat this submission as a business document, and submit it with your first and last real, legal name, and the first and last real names of your guests as it appears on your valid state issued ID and the ID of your guests. Full real names are required for the purpose of identity and birth date. No exceptions will be made.
Reserved Table(s) for You and Your Guests. You are allowed to have a maximum of 20 guests. Cake Table, and Gift Table Available upon Request. Complimentary Champagne for 21+, and/or Sparkling Cider for 18+, will be given to you in appreciation of your patronage
